4. Statement of good faith belief
A statement that you have a good faith belief that use of
the material in the manner complained of is not authorized
by the copyright owner, its agent, or the law.
5. Statement of accuracy
A statement that the information in the notification is
accurate, and under penalty of perjury, that you are
authorized to act on behalf of the owner of an exclusive
right that is allegedly infringed.
6. Physical or electronic signature
A physical or electronic signature of a person authorized to
act on behalf of the owner of an exclusive right that is
allegedly infringed.

Counter-Notification
If you believe that your content that was removed or to which access was disabled is not infringing, or that you have the authorization from the copyright owner, the copyright owner's agent, or pursuant to the law, to post and use the material, you may send a counter-notice containing the following information to our Copyright Agent:
1. Your physical or electronic signature
2. Identification of the content
Identification of the content that has been removed or to
which access has been disabled and the location at which the
content appeared before it was removed or disabled.
3. Statement under penalty of perjury
A statement that you have a good faith belief that the
content was removed or disabled as a result of mistake or
misidentification of the content.
4. Your contact information
Your name, address, telephone number, and email address.
5. Statement of consent to jurisdiction
A statement that you consent to the jurisdiction of the
court in which your address is located, or if your address
is outside the jurisdiction where Quantiorb is located, that
you consent to the jurisdiction of the courts in that
location, and that you will accept service of process from
the person who provided notification of the alleged
infringement.
Repeat Infringer Policy
In accordance with the DMCA and other applicable laws, Quantiorb has adopted a policy of terminating, in appropriate circumstances and at our sole discretion, users who are deemed to be repeat infringers. We may also, at our sole discretion, limit access to our website and services and terminate the accounts of any users who infringe any intellectual property rights of others, whether or not there is any repeat infringement.
Misrepresentations
Please be aware that under Section 512(f) of the DMCA, any person who knowingly materially misrepresents that material or activity is infringing, or that material or activity was removed or disabled by mistake or misidentification, may be subject to liability.
Processing of Notices
Upon receipt of a valid DMCA notice, we will:
Remove or disable access to the allegedly infringing material
Notify the user who posted the material that we have removed or disabled access to the material
Terminate repeat infringers in appropriate circumstances
Please note that we may forward the entire DMCA notice, including your personal information, to the user who posted the allegedly infringing content.
